Why is Community Support important after discharge?

​Community Supports can be the difference successful reintegration into one's community setting.

Having spent years in the field of rehab for those with DD, ABI/TBI, and other neurological disorders, I have come to the conclusion that extended institutional rehab is not always the most effective means for reintegrating back into society, or the greater community setting.
